What is a single tooth replacement implant and how is it different from other types of teeth replacements?
First of all, an implant does not require us to touch any other teeth. If you're missing one tooth and hopefully have enough bone support, we place an implant, which is essentially a screw that replaces the root of the tooth. There are two other parts that replace the rest of the tooth structure, so you can have a complete tooth. The implant goes inside the bone and supports the crown portion of the tooth, which is what you see. You don't see the implant because it's inside the body. It does not require any attachments to adjacent teeth. The teeth are attached only to provide support for the gum underneath, preventing food from getting through. If you're considering other options, you can get a three-crown bridge to replace one tooth, which would require a crown on the tooth to the right and left of it, or in front and in the back, to hold the tooth in the center. Another option is a removable denture supported by surrounding teeth, replacing the missing tooth. The downside of this is you have to take it out at night, and it's a loose item in your mouth, which isn't convenient.
Can you explain the full process of placing the implant from beginning to end?
The first step is, of course, removing a broken tooth that would be replaced by an implant later. After removing the broken tooth and clearing the site, sometimes the implant can be placed immediately. On the same day as you remove the tooth, you might get some bone grafting and place the implant, done by either your dentist or a surgeon. Once the surgical process is complete, the tooth is usually uncovered, and a special element is placed to help heal and create the gum tissue around it. After this, the second part screws into the implant and holds the crown, which is then placed. The time it takes depends on whether the implant can be placed the same day as the extraction. If it can, it takes less time; within six months, you can load that implant with a crown. If the procedures are separate, it takes at least two to six months for healing before placing the implant, and another four to six months to put a crown on the implant.
What is a bone graft and when is it necessary?
When you pull a tooth, there is usually enough bone support. The bone is wide around the root of the tooth, with bone in front and back. If it's a front tooth, and if it's a side tooth, you have bone on the cheek side and the palate or tongue side. When you pull the tooth and don't replace the bone, it collapses into the hole, losing up to 40% of the bone height, which is impossible to regain. To avoid losing bone height and width, which are necessary for the implant, you need bone grafting material. The graft can be synthetic, natural, from you, or not from you. This material is placed in the socket or hole that holds the root, ideally at the time of tooth extraction. Although it can be done later, it is much more difficult for the body to accept if done later.
What should patients expect in terms of healing after the implant is placed?
Most people are afraid of it, as they don't like dentists or drilling into the bone, thinking it will be painful. The truth is, it is almost not painful at all. It feels similar to when you were little, and your baby teeth started moving—a little twitch in your gum. You're supposed to keep it clean by brushing around it, and the discomfort goes away quickly. Do you need medication? Listen to your doctor, as each case is slightly different. Often, you don't need to take medication, and it will heal. The doctor may want you to take antibiotics, depending on the condition of your mouth, apart from the implant area.
What are the benefits of single tooth replacement with implants?
The primary benefit is replacing the missing tooth, which helps preserve the bone around it. You use it just like your own tooth. Another benefit is that an implant cannot decay because it's not an organic structure. It also looks cosmetically decent, so it appears you're not missing any teeth. It's an excellent product.
